Looks like too much throw in the tail rotor pitch slider. As he banged in the rudder for the turn, it over-traveled and seized up on/deflected too far and looks like it caught on the vertical fin or something. The tail rotor locks up solid and one blade goes flying off.
With no more tail control, the only option he had was throttle hold and try to autorotate. Not a lot of time to think it through when you are that low. It needs to be automatic. The only way to do that is practice, practice, practice. It should be rebuildable
